A right-to-left shunt occurs when deoxygenated blood from the right side of the heart mixes with oxygen-rich blood from the left side of the heart before it reaches the lungs, while a left-to-right shunt occurs when oxygen-rich blood from the left side of the heart mixes with deoxygenated blood from the right side of the heart before it leaves the lungs.
